Instructions. This is a fast OPAMP, to keep it stable, ensure good rail decoupling. Place a small capacitor of 0.1 to 0.33 uF and 5mm or 10mm lead spacing from pin 8 (V+) to nearby GND, and from pin 4 (V-) also to nearby GND. By nearby we mean preferably under the chip itself, and 50 mm is way too long.
The blue trimmer is for DC null adjustment. We have already adjusted it to 5mV max, however in case you want to readjust it, that's what it's there for. Connect a multimeter with the red wire on output of the OPAMP, then the black wire to signal GND. Then use a small screw driver to adjust the reading to 0mV or as close as you can.
